Registration Packets

Adult Client	This packet provides the forms that should be needed ahead of time for a new adult client. It contains the Registration form and Release to Physician. Other forms can be filled out with your therapist.
Child Client	This packet provides the forms that should be needed ahead of time for a new child client. It contains the Registration Form, Child Background Form and Release to Physician. Other forms can be filled out with your therapist.

 

Forms

 

Release Professional	Complete this form if you would like to allow your counselor to discuss your treatment with any type of professional who is not a physician or a pastor (see below).
Release to Physician	Complete this form if you would like to allow your counselor to discuss your treatment with your primary care physician.
Release to Family/Pastor	Complete this form if you would like to allow your counselor to discuss your treatment with your pastor or a family member.
Email Consent	Complete this form to give us permission to interact with you via email. (Note, by sending us an email message you are giving us consent to reply to you via email).
Record Consent	Complete this form to give your counselor permission to record your sessions and for supervisors to oversee your treatment.
School Observation	Complete this form to give our staff permission to observe your child in his or her classroom.
